Launch of Progress M-12M (44P) aboard Soyuz-U rocket from Area 1/5, Baikonur Cosmodrome, Aug 2011. Progress M-12M was supposed to deliver supplies to the International Space Station, but the Soyuz-U’s Blok I third stage suffered a malfunction 5m 25s into the flight. The Progress spacecraft failed to achieve orbit and burned up in the atmosphere over Russia.

The launch failure was traced to a gas generator malfunction caused by a one-off occurrence of human error during launch vehicle processing. The next Soyuz-U rocket successfully launched Progress M-13M (45P) to the ISS in Oct 2011.
